DESCRIPTION: The idea for this form came from collaboration with a city government.
The accompanying work flow is designed for the call center or receptionist of your city government, or large business, or it is also designed for a civilian to enter in the report from home. The information will be received and entered into the form. The form, based on needs and responses, may be moved through the work flow in many different ways. The work flow has also been design to prevent forms from "falling through the cracks" those responsible are reminded upon inactivity.
