INTRODUCTION: For those organizations with a disciplinary process we have created this form and workflow. This can be a very important process for many organizations. We have tried to create a general outline while giving enough detail to demonstrate the uses for this form.

DESCRIPTION: This form and workflow was created as a tool for employee disciplinary action. Every successful industry needs discipline with their employees and action should be taken when discipline is negated. This generalized workflow covers actions such as Verbal Warning, Written Warning, Probation, and Suspension. The form covers General Information, Descriptions, Witnesses to an event, and actions to be taken.
