INTRODUCTION: This form gives a generalized drug test consent form with legal documentation accepted by most states in the U.S.A. It contains company disclaimers and check boxes for the tests agreed to and includes date and signature of the employee.

DESCRIPTION: This is a generalized consent form used to grant permission to do simple drug and alcohol screening tests. It discloses the rights of the individual and explains what tests are required. Within the form is a paragraph saying; "I, the aforementioned, do willingly consent and hereby give my authorization to perform analyti­cal tests deemed necessary to determine the absence or the presence of alcohol and/or drugs in the following areas: blood, urine and breath." It also states what is expected from the company for any employee unwilling to take such a test. The form leaves room to bind it as a contract at the bottom with a digital signature.
