You can load sso /d which will spawn a new form fill debug screen that will give some useful information like whether the URL going through iChain matches the profile information, LDAP communication information, and Secret Store error codes returned. You could otherwise get a sniffer trace to confirm that the iChain box is forwarding the correct information to the browser to redirect to the application.
Many of the fill issues are related to javascript code not being executed - if you remove the /post tag , leave the /fill and manually submit the data (assuming that it is filled correctly), then the javascript tags that iChain 2.2 introduced will probably address the issue - check the iChain 2.2 documentation for details.
