Introduction
Would your company like to host an event with some type of entertainment? This simple form allows you to record artist information for use during just such an event.

Description
This form is useful for recording information about potential entertainment artists. It includes information about the group size of the entertainment as well as cost, housing, contact information, and other necessary details. Your entertainment and hosting assignments just got that much easier!
This form does not come with a ready-made workflow. It can be used with any approval workflow already provided on the Resource Library, or your company can created a simple workflow to manage the form.
