Bid List Application
INTRODUCTION: This form allows businesses that want to bid on and sell various commodities within a city's jurisdiction to submit vendor applications to their city government for approval and licensing.

DESCRIPTION: This customizable form template requires a business that wants to apply for vendor rights to provide city authorities with all data required to issue a license. The applicant's name, date of application, and contact information are listed first on the form, followed by the type of business that is requesting vendor rights (Individual, Partnership, Corporation, or Other). The applicant is then required to provide the applicable Tax ID # / Social Security #, as well as the names of authorized officers within the company. This sensitive information can be protected by customizing access controls within the workflow to ensure that only authorized officials have access to the protected data. Finally, the applicant is required to enter all of the applicable commodity codes in order to receive approval for the correct vending license. The applicant then submits the completed form.
When this information has been entered and submitted, the form moves to the proper city authority for review. The Reviewer clicks COMMENT in the upper left portion of the window and enters his or her name, selects either Approve or Reject, and provides the reason for the decision. The city authority then clicks OK at the bottom of the comment form and selects the appropriate action at the bottom of the window. Notification of the decision is sent to the business as well as to the department responsible for issuing the vendor license, and a copy of the application is sent to city records.
The workflow can also be customized to notify city or business officials of any further action that must be taken to complete the licensing process.
