System initialization fails after upgrade to Vibe 4.0 with LazyInitializationException

During the initial startup of a Vibe 4.0 system immediately after an upgrade from Vibe 3.x; the system initialization fails when trying to upgrade team memberships. The following error is logged in the appserver.log file:

Upgrading team membership for binder: Corp_Marketing (/Workspaces/Global Workspaces/NWT/Corp_Marketing )
2015-07-07 09:16:05,850 ERROR [localhost-startStop-1] [org.hibernate.LazyInitializationException] - org.hibernate.LazyInitializationException: failed to lazily initialize a collection of role: org.kablink.teaming.domain.Binder.definitions, no session or session was closed
